Help! LTO issues on CentOS, Can't Figure it out

Hi guys, frequent lurker but new member, I set up a new LTO server/4-drive robot for our backups, and am running into a strange problem.
I'm able to write to the tape and list it's contents, erase the tape, but I am not able to mount it as ltfs.

This is a new build: Hardware = Supermicro Server, SAS-3 3108, MagStor 4048 Robot, IBM Ultrium-HH7 Tape Drives (G9Q1)
